FAQs
What should I wear to my exam?
A T-shirt and shorts is great, but there is no required dress. You will not be asked to undress for your exam.
What if I need additional documentation from my PCP or specialist?
We can help you obtain those documents! Please text us prior to your appointment so we can make the necessary requests.
Will I get my medical card the same day?
Yes! You will receive your long-form and a laminated medical card at your appointment if you pass the exam. Occasionally, we need further documentation prior to providing the certification, if this happens, we will work with your to get your card as fast as possible!
Where is your office?
We have a small private office at 1500 Northwest BLVD in CDA. However, we are a mobile operation and can come to your home or business to conduct the medical exam.
Can I walk-in for an appointment?
We are appointment based only, which ensures no waiting in lines at the urgent care or doctor's office! If you do not see an appointment that fits your schedule, feel free to text us-- we can often accomodate after hours requests!
Do you do urine drug screens?
No, we do not provide any services except for the medical exam. Part of the medical exam is a simple urinalysis, which tests the urine for blood, sugar, and protein. We do not have the capability to perform urine drug screens or breath alcohol tests.
Can I use the eScreen Passport my employer gave me?
No. We are an independent, small business and do not contract with common DOT platforms such as eScreen and FormFox. We can provide you with a receipt to submit for employer reimbursement, or can invoice your employer directly.
